You won't see this mask or the private browsing mode text if you are in permanent Private Browsing mode.

  • Tools > Options > Privacy > Firefox will: "Use custom settings for history": "Always use Private Browsing mode"

You can make the title bar visible via the "Title Bar" button at the bottom left in the Customize palette window

  • "3-bar" Firefox menu button > Customize

This has covered what I had issue with, I use the 'In Private' feature all of the time. Before it would show the 'Mask' in the title bar, then suddenly it was not there any longer. I am very security conscious and was concerned I may have done something wrong/left something unset.

I'm here in January 2015 running 34.0.5 and the mask has disappeared, irrespective of what I do. In that area of Settings I DO NOT have the "Always use Private Browsing mode" item ticked, although I do have SOME of the indented items under that heading ticked. And that's under the major sub-heading of History. Does 'some' custom settings constitute "Always use Private Browsing mode", for the purposes of this question? No mask; no "Always use Private Browsing mode". PhredE 12th Jan'15
