How can I get rid of 'Ask' in the list of search engines, which always comes back (and at the top of the list) even though I check the 'Remove' box in 'Manage Search Engines'?

You can open the about:config page via the location bar and do a search for ask.com via the Filter at the top of that page.
You can reset all ask.com related prefs via the right-click context menu to their default values.

To open the about:config page, type about:config in the location (address) bar and press the "Enter" key, just like you type the url of a website to open a website.
If you see a warning then you can confirm that you want to access that page.

Unfortunately Ask has returned this morning and again seized first position in my search engine list! Yesterday I reset all Ask preferences to default and it disappeared from the search engine list. Today all entries had returned to user set. I tried to return them all to default again, but five do not respond and remain at 'user set'. Four of these are extensions and perhaps don't matter, but one is 'browser.search.order.1' - which I guess is the main culprit.
