When I press + to open a new tab, for some reason it is blocked from opening - same if I go to File -> New Tab.

This problem is being caused by the Ask Toolbar extension. Please remove or uninstall it. It has some compatibility issue with Firefox. For help on uninstalling it, see this article: Disable or remove Add-ons.
