why do filters run vs not run?

So I change the Message Filters. When finished, click "Run Now"

Some of the time, the filters run and implement the change I just made (and presumably run all of the other filters also). Some of the time nothing happens.

I'm sure there is a pattern, but I don't see it.

Would appreciate an explanation (or pointer to information) that explains why.

the run now button is on a line with the account and folder that the filter will run on. It also only runs the selected filter from the list above.
