I installed version 45.1.1 (83.67mb). Should I remove version 38.5.0 (159.37mb)? Since the older version is a larger file, I'm reluctant to remove it.

How did you install the new version? Install or update?

Windows seems prone to this problem of installing a new version alongside the old. Ideally it would overwrite the new over the old. Keeping both copies might be an aspect of restore points and the ability to revert to an earlier configuration.

Windows 7, SP1, 64 bit. It was installed as an update.

It will be a windows installer list problem. You will actually only have one version of Thunderbird installed.
