In Settings, "Always ask you where to save files" is check, yet never asks.
Why doesn't Firefox ask me where to save a potential file when in Settings, "Always ask you where to save files" is checked?

It may depend on the scenario and file type, see https://support.mozilla.org/questions/1389485 — does that sound it might apply to your situation, or you just get no prompts, no matter the associations and apps set to trust or open file types etc.?
