Unable to add MFA

I just wanted to preface this with I am not a beginner to using MFA. After I scan the firefox QR code into DUO, MS Authenticator or google authenticator the pass to verify never works, I just get the red box stating 'Invalid two-step authentication code'. I have tried verifying with many rescans of the QR code in all 3 apps.

If it's not clear this is trying to add MFA to my firefox account I sync my bookmarks etc with.

If you scan the QR code then it should normally work.

Make sure that the time on both devices is correct because MFA (2FA) works with time dependent codes.
