Use the Reset toolbars and controls option in the Use Troubleshoot Mode in Firefox dialog to restore the default toolbar layout: https://support.mozilla.com/en-US/kb/Safe+Mode#Safe_Mode_window

If that doesn't work see if the problem occurs in Use Troubleshoot Mode in Firefox
