I've toggled two-step authentication off/on multiple times, get new set of recovery codes, but I never get the QR coded, how do I generate this, please advise...thanks!

Did you scan the provided QR code with an authenticator app (e.g. Google Authenticator) on the mobile device ?

That's the problem, I don't get the QR code, how do I generate it ?

You should automatically get a pop-up with the QR code once you enable 2FA.

If you use extensions ("3-bar" menu button or Tools -> Add-ons -> Extensions) that can block content (Adblock Plus, NoScript, DuckDuckGo PE, Disconnect, Ghostery, Privacy Badger, uBlock Origin) always make sure such extensions do not block content.
