Firefox 4 and later start in Save mode if the Shift key is pressed while you click the desktop shortcut or if you use a shortcut key that only has a Shift key modifier.
If the latter applies then use a keyboard shortcut that uses other modifier keys like Ctrl or Alt in addition to the Shift key.
