How do I get Auto Insert of name, email etc to work again. Is there a plug in conflict?

Running FF 3.6.8

The auto insert of name, email, etc has quit working on sites I visit.

How do I get it to work again? Is there a plugin conflict?

See Tools > Options > Privacy > History: "Remember search and form history"
