Private Browsing, do not remember history, do not track...still have history after restart. Not good.

I see that you have the Private Tab extension. Are you sure that the history is from tabs that were in Private Browsing mode?
Did you remove all the history in the History Manager?
You can check for problems with the places.sqlite database file in the Firefox profile folder.
