I checked your Firefox 12.0 Crash Report [@ js_GetIndexFromBytecode ] 

https://crash-stats.mozilla.com/report/index/6d5e2774-a824-4868-b42c-ecd6c2120601  and related bug 700176.  The crashes may be caused by Trusteer Rapport software (RapportTanzan**.dll) so either update that software or disable it, if restarting in Firefox Diagnose Firefox issues using Troubleshoot Mode doesn't help.

How are you trying to load Safe Mode -- holding the Shift key when you start Firefox is one way you might be able to do it.
