The previous version had a small tab in the tab ribbon that would open a new tab.

There is a temporary work around. Please search for the classic theme restorer in addons.mozilla.org.

I've installed the classic theme add on. There doesn't seem to be a way under the Tabs settings to add the 'new tab' in the tab ribbon. Can you provide some further details on the work around? Thanks

I'm not sure what you mean with the 'small tab'?

Firefox has by default a plus (+) button on the Tab bar that changes to a New Tab button when moved to another toolbar. If you previously had another button on the Tab bar then it was added by an extension. If you lost the plus button then click Restore Defaults in the Customize palette window.

No issue with the (+). The 'small tab' was on the tab ribbon to the right of an active tab. If the 'miniature' tab (a blank or ghost tab?) was clicked on it would open a new tab (just like what the (+) now does, and a new small tab was created like the last. Best shown by the attached pic of IE11 as this has, as default, the working principle I'm referring to.
