How to format password data CVS file to be imported into firefox?

We have been unable to find an explanation of what components of the password file data, like created & modified date, can be included nor any formatting requirements. We've read that only the url, username & password are necessary... does that imply that the date/time fields would not be included if provided when imported?

Following the format of an Exported CSV password file seems reasonable... but risky as it could corrupt the existing file?

I've never tried it, but why not export and import using another version of Firefox(Beta, Dev, or Nightly) to test.
