In the main menu,
File|New|Existing Mail Account
In the Application menu, it's hidden behind New Message.
Alternatively use any route that gets you to Account Settings (e.g. right-click an account, or go to Tools|Account Settings in the main menu, etc) and then click the Account Actions button down at the bottom of the pane listing your accounts.
Now you have got to a dialogue that lets you add an account to Thunderbird, assuming you have already registered an account in your name with an email provider.
