One possibility is having the SMTP server with a different email id than the account's email id. you can easily check this with these steps:

  1. click to 'account settings'
  2. highlight account name in left sidebar
  3. now, look in lower right where a button says something such as 'edit SMTP server.' click that and compare that email address to the account address.
