Chosen solution

You could try the Customize feature. This will show all the available buttons that are not on any bar at the moment, and allow you to move it back to its normal location if you see it on another bar. If all else fails, there is a "Restore defaults" button at the bottom of the customize panel to go back to the "factory" toolbar layout and then you can customize from scratch.

In my Firefox 52.0.1, the plus sign "Open a new tab" was wisible only when I opened the "Customise" tab. When I closed it, the icon dissmissed again. At last, I moved the icon to the left side in front of the first tab. At this place it survived even when I closed the Customise Firefox tab. Then I customised it again and moved it back to the right, and everything is O.K. now.
