To open a new tab you can use ctl+t or the symbol in the shape of a vertical cross on the tab bar.
Are you using userChrome.css in the chrome folder in the Firefox profile folder that may need a update ?

I'm not sure what you are trying to say. Do you want to add a Firefox icon to your main Windows Taskbar? If so, simply right-click on the Firefox shortcut icon and then left-click "Pin to taskbar".
Do you want to add a button on a toolbar inside Firefox which opens a new tab? If so, simply right-click on a Firefox toolbar and left-click "Customize Toolbar...". Look for the "Open a new tab" icon in the customize tab. Drag and drop it somewhere on your toolbar.
