Open in a new window is no longer usable in v4.0.1

After installing v4.0.1 the open in a new window function no longer works properly. A new window is opened, but where the bookmarks toolbar should be displayed it shows a blank line. The navigation toolbar is displayed, but does not function. If I click on links in the displayed page the URL in the navigation toolbar does not change to reflect the new page I am seeing. The reload button in the navigation toolbar will not reload the page.

Try:

Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).
