Hi, you can report them from Help --> Report Deceptive Site as you are the one with the info and addresses and are 1st hand party to it. Also : https://safebrowsing.google.com/safebrowsing/report_phish/?hl=en

Please give yourself a scan just to make sure : https://www.malwarebytes.com/

This scam claiming you can get a Samsung S8 or S9 or iPhoneX or $1000 gift card for just doing a survey and or a money transfer of $1 have been occuring for some months now. The version of page and domain used has varied often times with a .loans domain.

Mozilla is the company that makes the desktop Firefox web browser so Firefox itself could not be doing a survey.

edit to add: Mozilla has done surveys over the years by way of http://qsurvey.mozilla.com which is surveygizmo.com however no form of prizes are given for doing them.
