Why is the "When new messages arrive: Play a sound" option so erratic?

Sometimes when I get new mail, my custom sound plays and sometimes when I get new mail my sound doesn't play at all. Silence. Can we get the wrinkles out of this?

Also, why can't I set a different customized sound for each of my many email accounts when new messages arrive for each of them?
