Firefox changes the page size to index card when I open the print menu. Safari does not. The defaults are set to US Letter. Any help?

My printer settings are set to US Letter. When I try to print from Firefox it comes up as Index Card Letter. I cannot change it in the print menu so I go to page setup and change it, then back to print menu. This does not happen in Safari. I have an HP Office Jet 4650. I can print using the Index Card setting, but it cuts off the top of the first sheet. Also, I get a warning notice when I try to print two sided.

The first time I tried it I did not change the page setup before I did the printer reset. The second time I did change the page setup and it reopened with the correct printer setting.
