Are you sure that you visited that site in a Private Browsing tab?
I have just opened a new Private browsing tab, visited a website (https://arstechnica.com) and the site does not appear on my homescreen or in my browsing history.
I am 100% sure. It did not happen just once, I have noticed it few times.
