Custom toolbar button won't show up?

I have tried to create a custom toolbar button by following the steps of this guide: https://developer.mozilla.org/en-US/docs/Mozilla/Tech/XUL/Toolbars/Custom_toolbar_button I have gone through the first ten steps but after restarting the application my button won't show up (It's not in Customize Firefox tab). I'm pretty sure I've copy-pasted everything just right and the filenames are good too. But is there anything else that could cause the problem?

This might not be applicable to Firefox 57. Although the code of the toolbar is roughly the same, the way you build extensions has changed. If you take a look at the buttons on this page, you could get a sense of how to create a command button in Firefox 57:

To view the code of an extension on the Add-ons site, you can right-click the Install button and use Save Link As. Any program that can open a ZIP archive can extract the .xpi file, but you probably knew that already.
