First, you should update your version of Firefox to 15.0.1. Update Firefox to the latest release

Then, read Control whether Firefox automatically fills in forms

Make sure that you do not run Firefox in (permanent) Private Browsing mode.

  • Tools > Options > Privacy: Use custom settings for history
  • Deselect: [ ] "Always use private browsing mode"
