when installing I clicked options and made sure the box that says, install firefox in the start menu was clicked but no matter how many times I install the folder in the start menu doesnt populate, I can still open firefox and use it as there is a icon short cut on the desktop but after a while this ico will break and change to a broken Icon even if I sent to tast bar

Did you try to create a shortcut manually with Windows Explorer in the start menu folder?

This can be in the "All Users\Start Menu" folder or in the "Start Menu" folder of a specific user.
