How do I place a Reload button on my toolbar?

After I installed FF 7.0.1, the handy reload button disappeared. I can reload individual tabs, but not the main screen when it is the only screen up.

The Customize toolbar info doesn't address this. Can I add a Reload button? How?

Somewhere FF states that "reload is now in the address field" but on the main screen that spot does not show up in mouseover. Where do I find this area?
