I see PageRage Toolbar 2.7.1.3 listing among your Extensions under "Troubleshooting Information" in your post.

Have your tried to uninstall it using Tools > Addons > Extensions? See: http://support.mozilla.com/en-US/kb/Uninstalling+add-ons#How_to_uninstall_extensions_and_themes

See the following; scroll down to "How do I uninstall Yontoo Layers?" and "How do I uninstall PageRage Toolbar?" and click on each of them. It appears Yontoo and PageRage are related. Remove both through Start > Settings > Control Panel > Add or Remove Programs with Firefox closed. Print the instructions before closing Firefox so that you have the instructions in front of you:

Be careful what you download and install on your system; think twice before doing so!

Its easy! Just go to tools, add ons and then click extensions and there are all the toolbars!
