Do you notice this problem with any particular kind of file?

Firefox uses a header sent from the web server to assign actions to different types of downloads. For example, the server might send application/pdf for a PDF document or text/csv for a comma-separated values file which typically opens in a spreadsheet program. Firefox actually ignores the file extension itself, which could be .pdf, .php, .aspx or whatever.

You often can review and confirm or change these settings here:

orange Firefox button or classic Tools menu > Options > Applications

Edit: If the relevant file type is not listed on that tab, you might have to do the full reset in the above answer, or you might be able to delete just the particular settings file for MIME types.

The instructions in Reset Firefox preferences to troubleshoot and fix problems explain how to reset all your preferences back to the defaults.
