On the tab bar, there was a tab with a "+" sign on it to open a new tab. It disappeared and I've reinstalled the 4.0 update twice and it hasn't come back.

Try:

Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the add-ons is causing the problem (switch to the DEFAULT theme: Tools > Add-ons > Themes).

  • Don't make any changes on the Safe mode start window.

I totally uninstalled Firefox instead of reinstalling the update. That fixed the problem but I still don't know why it went away in the first place.
