I set ff4 so that the add-on bar is not visible, but every time I open a new tab it reappears - not just in the new tab but in all tabs.

This is probably caused by the McAfee Site Advisor add-on, other users have reported that it has caused this problem. To test that try disabling the Site Advisor to see if the problem goes away.
