Pages not being refreshed on firefox startup

With a fresh install of 16.0.2, pages restored from "show my pages and tabs from last time" do not refresh when Firefox is started. The problem does not clear starting in safe mode. browser.cache.check_doc_frequency has been set to 0 and 1 with no change in the behavior.

The only "solution" I have found is to set browser.disk.cache.enable to false, but that is counterproductive in the end.
