It says tap the padlock on menu bar to access site permissions. However there is no padlock. Just a shield with checkmark saying that enhanced tracking is on.

Oh, the problem is on Android? The lock icon is a feature of "desktop" versions of Firefox (Windows, Mac, Linux).

On Android, if you have already customized a permission for the current site, it will be listed on the Tracking Protection panel. If you do not see a customized permission listed there, then Firefox will apply the general permission set here:

menu button > Settings > [Privacy and security section] Site settings > Camera

If that one is set to "Ask to allow" by default so the site can request access to your camera and you can grant/deny.

Is the DocuSign site not prompting for permission? Does it work on other sites?
