How can I add text and check boxes in a Pdf file?
I was able to add text and check the boxes on my pdf file but when I sent it after saving it the complete form was blank! Nothing I had added was there and the form when back to being blank even on the file I saved after putting/typing in my information.

Not a Firefox issue. Firefox allows you to view PDF files as "read only", it doesn't include features for editing or saving. You need to use a complete PDF program to get those types of features.
Firefox's built-in PDF viewer does not have all the features of Adobe's plugin or the stand-alone Adobe Reader. Especially script-driven or interactive features such as form submission may be underdeveloped.
If you do not already have Adobe Reader on your system, you can download it here:
If you prefer the earlier version (Adobe Reader XI) you can use this page:
https://get.adobe.com/reader/enterprise/
After installation, please use the Adobe Reader Help menu, "Check for Updates" to retrieve recent security updates that weren't included in the main download.
Then finally, when you open a PDF in the viewer that requires more advanced capabilities, you can click the download arrow along the narrow PDF viewer toolbar to open it in Adobe.
But... as Ed notes, not all PDFs allow you to save them with your added data. This is something that the form author has to enable. Otherwise, you might only be able to save your input as a data file rather than in the PDF itself.
