Next to the last tab I had a + that I could click to open a new tab. The + has gone, here can I restore it?

Would you try this please :

Preferences => Customize and see if the plus sign is there. If it is, drag it to where you want it to be. If that's not possible, click 'Restore Defaults' and see if that will bring it back.

If everything else fails (I hope not  !), there is this add-on :
