Mail Merge Problems with Thunderbird

I followed the mail merge instructions last Christmas and had no problems. This year, I can't get anything in mail merge to work. First, I downloaded the latest TB and mail merge add-on versions. When I try to use mail merge, first I don't see the "Format" box that's shown in all the instructions. Secondly, when I use the .csv file I used last year (when it worked), I get an error message that is unintelligible to me, so it doesn't help me in solving or addressing the problem. The error is: [Exception... "Component returned failure code: 0x8050000e (NS_ERROR_ILLEGAL_INPUT) [nsIConverterInputStream.readString]" nsresult: "0x8050000e (NS_ERROR_ILLEGAL_INPUT)" location: "JS frame :: chrome://mailmerge/content/utils.js :: csv :: line 365" data: no]. When I use a smaller subset of the data, it doesn't work either, so I don't know where to begin. I am simply trying to forward a Holiday ecard to our customers, but mail merge does not create emails with the image of the card at all. Any ideas?
