flash changes activation status

I have set the Adobe Shockwave Flash player to 'Ask to Activate', this works for a time, but then it returns to 'Always Active' status.

I thought at first I might have when activating it (after clicking on the 'Activate Adobe Flash' and then getting the popup with the two choices 'Allow Now' or 'Allow and Remember') chosen the Allow and Remember option by mistake at some point but after this continuously happening I am very sure that I have not made this mistake.

To make matters worse, it does not happen all the time. I can watch a few Flash movies and the status does not change, then at some point I go to a site and the flash videos start on their own and when I check the status it has reverted to 'Always Active'.

I have used this option for years and until recently I have not had to reset it back to 'Ask to Activate'. Somehow between an update with Shockwave Flash and/or Firefox I am now constantly doing this.

Please help me so that I am not constantly resetting this option back to 'Ask to Activate' which is how I wish this to function.
