To add or remove items from toolbar you must be in customize menu, Right-click an empty section of the Tab Strip and select Customize...

if the above does not solve the issue then try to delete the localstore.rdf file in profile folder

beware do not delete the localstore.rdf file in the Firefox program installation folder
