Firefox addons won't appear - toolbar customisings restored.

Sometimes Firefox addons icons will appear, sometimes they won't. When I restart Firefox, all my toolbar customisings are reseted - they are back in the default set. Addons icons are gone from the Navigation toolbar and the Customise Toolbar view. Despite of that, addons are working - for example Flashblock and Adblock are on and I can't get them off. I have 13 different addons.

I have reinstall the Firefox. Is the problem Firefox or the operating system, Windows XP home?
