Any time I should see a "cant' connect to server screen" a page loads with multiple links to instantfwding.com. I have tried uninstalling, deleting all data in %AppData%, I have run multiple malware scanners, and antivirus scanners, but have had no success. Google search shows multiple references to this "instantfwding.com" redirect issue, but no clear method for resolution. Has anyone else experienced this? Any ideas on how to fix?

It's a form of malware, possibly in a plugin, possibly installed as bloatware with something else, could even be in your router, hard to know exactly without more debugging.

Try a refresh of FF: https://support.mozilla.org/en-US/kb/refresh-firefox-reset-add-ons-and-settings

You'll have to reinstall your favorite plugins, etc.

[If you really want to be thorough, export your bookmarks, wipe FF entirely, reboot, and reinstall.]
