The toolbar(?) at the top of the page that lists File, Edit etc. is not displaying. How do I get this do display?

Just right click on the top of the firefox window and select the Menu bar option.

You can't hide the menu bar on a Mac via the toolbar context menu

What do you see if you open Firefox?

Are you running Firefox in Full Screen mode?
Try command+Shift+F
