Someone called me today to get help enabling cookies in Firefox 10 on Windows.

On the Privacy tab we had selected Use custom settings for history but the option to accept cookies was grayed out. The drop-down box was selected to Keep until I close Firefox, but we could not change it since it was grayed out.

Private browsing was not enabled, what would cause the ability to enable cookies to be grayed out?

A security application or anti-malware application on that PC may have taken over the handing of cookies from Firefox and is preventing those prefs in Firefox from being available to the user.

Do any prefs show as locked if you click the Status header to sort by status?

Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the extensions or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).
