Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the extensions or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).

What leads you to think that your browsing history is being saved?

Because it is in the browsing history. Even after closing FireFox and restarting.

Here's some more info...

I can "manually" start private browsing for a session but as soon as I check "always use private browsing" in the tools-> privacy settings, it goes back to standard browsing and begins saving my history.

Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the extensions or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).

I don't know how but simply starting Firefox in safe mode fixed it. I didn't change anything... Go figure.
