Using Metacity does not make any difference.

Toggle full screen mode with the F11 key a few times to see if that brings the title bar back.

Open the system menu via Alt+Space and see if you can resize that window.
If that works then close Firefox to save that setting.

Thanks very much cor-el, that did the trick. It was driving me crazy. Just reinstalled Linux Mint9 and re-configuring things and thought I might have to re-install again! You saved me lot's of time. Thanks
