This issue can be caused by an extension that isn't working properly.
Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).
- Don't make any changes on the Safe mode start window.

Many thanks for your help. In safe mode all the tab labels & icons reappeared. I restarted in normal mode and all tabs were again properly labelled with icons; yet I made no changes.......
