cannot login after one unsuccessful attempt into phpmyadmin

I have come across an issue when attempting to login to phpMyAdmin on two different servers. In Firefox, if I login successfully the first time, then I can logout and back without problem. If I type either my username or password incorrectly and I get the alert telling me that my login was incorrect then I cannot log onto that phpMyAdmin unless I completely close out of all Firefox windows and restart Firefox. This issue does not occur in Chrome, RockMelt or IE. So I do not believe the issue is with phpMyAdmin or my servers.

This issue occurred in Firefox 3.6 as well as 4.0.

Below are the two versions of phpMyAdmin that I am running. They are both running on Ubuntu Server 8.04 LTS
