pages not updating between visits

When I browse frequently updated websites it shows the page I saw last time I visited . When I press refresh it updates the page to the live version. It happens 100% of the time on BBC news for example since the recent big update.

This happens even if I fully quit or force quit Firefox between visits and I always close my tabs when I am done checking each site.
