Choose a calendar reminder action
I imported an appointment into the calendar. I wanted to setup a reminder. The only option is "Show an alert". Is it possible to to create a reminder option other than "Show and alert"?

You can edit the imported event, which gives you several reminder options.
I don't understand.
I have attached an image. I imported an .ics file. I can edit the entry but the only option I see is "Show an alert". I was hoping to be able to change the information to show what to do when the alert occurs. I need to do something a month prior to the reminder date.
I think that you've imported to a local calendar : a local calendar can not send email as reminder. In order to have email as an option of reminder you need a calendar with email ( like google, yahoo etc ).

I have an appointment. I imported the calendar event. I need to accomplish something prior to the appointment. I can set a reminder for 30 days prior for me to accomplish the task. When I setup the reminder I used the "custom" option. If you look at the image I provided there is an option "Choose a Reminder Action". When I saw that I thought I could create an action that would appear when the reminder appears. There is a down arrow which made me think there were options available.

The reminder action is 'Show an alert'. You can also choose not to show an alert, if this is what you want. 'Show an alert' is the only available reminder action, as Thunderbird does not send email reminders.

Fine. If you look at the image you provided there are multiple options for 'Reminder Details'. It has everything you need.
I was assuming that there was a way to enter a reminder that I was supposed to complete prior to the appointment date. In "Reminder Details" you can select either, a reminder for days, minutes, or hours, or you can us the second option and pick a date and time.
The next topic "Choose a Reminder Action" only has "Show an Alert" with a down arrow. I was hoping that there was a way to enter something that would remind me what I needed to accomplish prior to the date of the reminder.
