plugin check reporting Adobe Reader as outdated, vulnerable and a red Update Now button. Current version is 11.0.8.4.

however when you click on the button to update, the latest version is also 11.0.8 so it stays as vulnerable after installation

This is because Adobe dropped support for Windows XP after 11.0.0.8. You will need to either update your OS, or find another PDF reader.
